×We are in process of implementation of New Hire Process in Jira Service Management and we will have a form having multiple fields but there would be some fields which should be only available when other field value is selected, Lets say we have a Department and Each Department would have x number of job tiles/Designations and each job title/destination would have a access template (text) which should be popped up in customer portal for hiring manager to see and there would be roughly around 450 unique job titles.
You can create a dynamic form using native functionalities called Forms. Here is the link to Atlassian documentation: click.

Hello, @Muhammad Adeel Rehman
To achieve the desired functionality in Jira Service Management, you can utilize forms that are published to the Jira Service Desk interface and augment their functionality with the add-on developed by my team - Smart Jira Forms.
With this add-on, you can add various custom fields and implement Form Logic. We have been using this in our company to set up form logic based on a person's position, ensuring that they only see the fields that are relevant to their role.
This aligns with your requirements, and we hope that our add-on will be useful in achieving your goal!
