Export a form with all fields present

We have a form we use as a checklist, and it would be great if we could export to PDF but when the fields aren't checked, and it shows all the fields. Is this possible?

Can you elaborate a bit more on what you're trying to achieve? 

Are you talking about a portal form?

Screenshots would be helpful.

The first screenshot is the form in the ticket but the second pic is when you choose to export it as a PDF and none of the fields are listed because they are not checked.
