I'm trying to create an intake form using JSM that replicates one in a different Jira cloud instance. The problem is trying to get the description field pre-populated with prompts for the customer to answer when they create their request.
I have looked everywhere for documentation on how to do this in JSM. I can find articles about it for Jira, but haven't found anything for how to do it in a JSM template.

you should be able to do this with a pro forma form, and not the built-in request form.

I'm not sure what you mean by pro forma form? I started with a blank form, but haven't figured out (despite lots of trial & error) how to put multiple, formatted fields into the form.
