How to add additional columns to Requests page in Jira Service Desk project say Assignee, Created Date and Due Date?

In case if you are trying to customize in agent view of application you can follow pramodh's answer.
If you are looking to add more columns in request page of help centre then you need to navigate to the Help Centre --> Requests(Right top next to profile) --> Then in right side of the screen you can see "Customize" button there you can add the fields like this:
