How to have a form like Jira's support request form

I'd like to have a request form like Jira's support form: https://support.atlassian.com/contact/#/

Is it possible natively on jira service cloud? Or is it possible with some (easy) customisation?

Not in Jira - the support page you have referred to is a simple bit of html that asks you a couple of questions about what you are asking and directs you to the right place for it (several of the landing points are service desk portals themselves)

I said it wasn't.  It's a page outside Service desk.
