my created service request type it doesn't appear in my service requests list

I copied the 'onboarding employee' service request and changed the name into 'off boarding employee' . When I try to use this service request type in 'create' it doesn't appear in the options, see picture. Also it doesn't appear in the portal either, see picture.

Thank you Manon for your suggestion.  Yes, the service request has restrictions. I added my name to it to give myself permission to use it.

Following what I said above, I double checked the permissions again and added my name. Now it works!
