We want to use JIRA Project in our IT Dept. We would buy a license for 25 users for IT. We want to create a form in JIRA Project that asks our business users some questions about the work they are asking us to perform for them. Do we need to buy a license that covers all 500 of the rest of our company, or can we create a form that non-users can fill out and submit? They will have no other use of JIRA other than this form.
If you need to track each user (for metrics purposes), you will then create IDs for each responses which will need licenses for each.
If no, just create a general accounts that anyone can use, then utilize the JIRA Issue collectors for what forms you like.
We will need to know who answered the survey. We'll have to go with getting user licenses for our business users. Good thing your product is affordable

Unless that dashboard was made Publicly accessible, they would need to have a licensed JIRA account.

Does "publicly accessible" mean anyone in our company? or anyone in the world? We want to make them accessible to anyone in our company.

If you're using the installed version of JIRA within your own environment (ex. installed on your own hardware), your network/security team could lock it down to just your network. If you are using the Atlassian Cloud, Public Access would make it available to anyone in the world; unfortunately it is not possible to whitelist the Cloud to just your environment.

Anyone who needs to access JIRA to create issues will need to be a licensed user. You may want to look at using JIRA Service Desk as Customers do not require licensing. Your "Customers" can be your company's employees and they can submit the form through a Portal.
