Duplicate items in project list

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!
Hello, could you please tell me why I have duplicated items in my project's list. After adding a Level 2 Task category Initiative, then linking an Epic to this Parent in my Plan section, going back to the Project list I can see the items duplicated. See screen shot.
NB I added my Company Setup Epic to the JDE Civil Migration Initiative in the Plan section as I couldnt do that with in the Project.
