Form not showing when raising request via the portal

Hello,

 

I've created a form and linked it to a request type. When going to the request type and previewing it I can see my form (help centre). However, if I try to raise a request from within Jira (Create Request), the form is not showing.

Any help please?

What you're describing is expected behavior, there are 3 locations for forms, issue creation in Jira is not one of them.

  • Request form: Customers will fill out this form when they raise a new request from the portal.
  • Create new issue: Use a link to this form to create new issues in Jira Service Management.
  • Recommend in issue: Show at the top of the list of available forms when adding a form to an existing issue.

You could enable the Recommend in issue setting, and then when Jira users/agents add a form to an existing issue it would be recommended at the top of the list of available forms.

Or alternatively enable the Create new issue setting, customize the link, and then direct your Jira users/agents to only create issues using that specific link. That way they'll always fill out the form and create the issue when submitting. Though at that point you may as well just direct them to the portal to fill out the form/create the issue.
