How can I adjust the screen layout in create view?

I want to move the fields to different positions in the CREATE view only (i.e. the screen I see when creating a new issue). 

How can I do that?

Go to Project settings > Issues > Screens, from here select the screen scheme that belongs to the issue type you want to rearrange the screen for. This will take you to the screens and from here you can go in and reorder, add, and remove fields. If you only see one screen for the default one you will have to create a new one and associate that one with the create action.
