How can I insert help text to Jira Service Desk form?

I would like to help my user to understand my question by

• help text for each field of my form. Not just a description, I really want to explain stuff, give samples, etc...
• pre-fill some textbox with a template of information required.

Is it possible to do this with Jira Service Desk or any other add-on?
