I have requirement where User needs a form where they need to accept agreements which is well document on "Confluence page".
Is there a plugin or custom field type available where I can add URL to Jira Create issue form?
I am using Jira Software Data center edition 7.7.2/

The Jira Toolkit Plugin provides a a few new custom field types.
One is Message Custom Field (for edit), which let's you include a message in the create issue screen that supports HTML, so you can include links.

You could add a custom field (Checkbox) to the screen being used by that project to create issues that contains the URL to the confluence page in the option of the checkbox field. I don't think it would be clickable though.

Thanks for suggestion, checklist worked in this case. One more question, is there a way to show/hide fields on Create issue form based on what user selected.
I am looking for more like either cascade checklist or option to show field "XYZ" if user selected option "A". And hide field "XYZ" and show field "ABC" if user selected option "B" before.

By default you only have cascading dropdowns. There are several plugins that provide additional custom fields that might work better than the cascading dropdown. You should look at Profields.
