I have 2 custom fields which I have added to the create bug report screen. They do not show up by default. I have them set to required and show in the field configuration. I can make them show up, but only if I go to the configure fields dropdown and select them.

There's not much information on what's it that you want to setup, but I think that it might relate to the View Issue screen operation.
Any field with a value that you make available in the View Issue screen operation will start to show up by default.
