Organization being removed on transition

I have an issue with the Organization being removed on a transition for no reason (no post functions etc that cause it)

The transition is a customer facing one called Customer review. If the customer accepts our resolution, they can see and press in the portal "Accept Resolution" if they don't accept our resolution, they can press in the portal "Reject Resolution".

Accept Resolution closes the ticket, also allowing the user to leave a common (on a screen).  (Error doesn't happen on this transition, organization remains)

Reject Resolution moves the ticket back to the start of the workflow, also allowing the user to leave a common (on a screen).  (Error: Organization gets removed on this step, regardless of if they leave a comment or not)

Please note no error message is given, nor does the Organization being removed show up in the history section on the ticket.

Jira Version is 7.11.2
Jira Service Desk Version is: 3.14.2

We didn't always have this issue, when we moved to 3.14.2 we started having this issue.

Also i'd like to note that i've looked throughout all the logs and can't find any errors or entrees that relate to the above.
