Can I import a word doc form into Jira form

I have a form created on word/pdf that is for a certain event, and the user is requesting that if that event happens to have this form embedded, attached, or some how added to our current form being used for this project. Ideally id like to not recreate this existing form in Jira Forms, and to import or copy it over somehow.
