I got an email notification stating it is past due date. But the status was closed 3 months ago. Was it a bug or do I miss any flag?

It might be that the notification is looking at the Resolution field, which might be empty even though the status is Done or Closed or whatever.
It is always a good idea to add a post function to all of your workflows to set the Resolution field to a value for all transitions going to Done category statuses.
