Selection of fields based on an option

Dear community,

I am creating a type of requirement that will be entered through the JSM self-service portal. I have the challenge of creating a series of fields to be completed, but these must be activated or appear based on a specific selection of a combobox with the options.

I thought about creating forms with the fields by option and that they are loaded when something is selected in the combo, but I don't know how to do it.

Do you have any step-by-step instructions on how to create something like this? A use case example would be very helpful to me in understanding how to create something like this.
