Something's gone wrong Key: Key is required. when submitting form via link

I created a form, sent the link to myself for testing, when i want to submit i get the message 

Something's gone wrong Key: Key is required

Its missing the project key but how to deal with this. 

Plz help.

did you add any additional validators in the create transition (within your workflow) of this issue type?
Can you also provide a screenshot of the configuration of your form?
