I am using the following hierarchy on Jira.
Initiatives
Epics
Tasks
Story
Although I have Epic as a child of Initiative and Tasks and Story's as childs of the Epic, on the main page the Tasks and Story's don't show as being child issues?
Why is this?

Only child issues are shown, not grandchildren issues. In other words, if you are on the Initiative screen, you will only see Epics under Child issues. If you are on the Epic screen, then you will see Stories and Tasks. And if you are on the Story or Task screen, then you will just see Sub-tasks.
Thanks for the information - that's a bit annoying as I was hoping the full hierarchy would show but thanks for confirming

As @John Funk mentioned, only the child issues are shown in that screen.
For viewing the entire hierarchy, you could try using the List view available in each Jira project -> this will allow you to see the entire hierarchy.
Another option would be to use a plugin from Atlassian Marketplace. In case you want to use a plugin our Great Gadgets app can display the issues from a filter / JQL by their hierarchy (including initiatives) in Jira dashboards.
