Time Spent Field Not Showing Up in Transition Screen

I created a transition screen so that when issues are transitioned in our workflow, people can add time spent.  However, the time spent field is not showing up.  First screen shot is what I need.  Second screen shot shows what comes up in the transition.  Third screen shot is how the screen is configured.  Any insight on this would be greatly appreciated.

I believe your configuration is correct. It might be worth checking if your screen has been added to the transition.

If all verifications are done, I hope someone else can identify the misconfiguration.

Thank you so much for the quick response.  The screen has been added to the transition.

I even tried to do a validation on it based on what I read in other posts and still not seeing it.  

If I should put in a support ticket, please let me know.  Thanks.
