User field with date field

Wondering if it's possible to have a user and date field associated?  Basically, I want to select a user in a field (User Picker (single user)) and associate a date (Date Picker) in a field.  The person selects the username and the date:

User:

john.doe@xyz.com

Review Date:

July 26, 2023

Or also have the ability to have a user field with an action field (Select List (single choice)).  The person selects the username and 1 of n choices:

User:	john.doe@xyz.com		Status:	Accepted
				Reviewed
				Rejected

These fields would need to be associated with each other to maintain integrity and an audit trail.

Hi Kian,

I'm trying to have 2 fields that link to each other.  The first would be a user field linked with a date field and the second would be a user field with a selection list.

The first example would be to be able to select a user and that user can select a date that they performed the task.

The second example would be to be able to select a user and that user make a selection from a drop down list.

These actions would need to appear side-by-side on a form.

I don't know of anything like that.. Have you thought about using Subtasks? You could create a subtask for a user (assign it to them) and then they could fill out the details you want to collect.
