User updates in summary mail show by Anonymous rather than the user name

A newly created user has reported that the summary emails of changes she has made in the ticket shows Anonymous user but when you review the ticket the correct user displays in the history of the ticket.

As far as I can tell this is isolated to this one user.

Any ideas, how to fix this?
