how can i show a note in the create screen??

hi

i need to show a note in the create screen.

i use the checkbox customfield and i write the note as a description but i have to set values for the checkbox custombield. So in the create screen i see the note but also the value of the checkbox becasuse they are together.

how can i only show a note in the create screen without using a checkbox or without the value of the checkbox , only the description of the field?
