show conditional textbox when any checkbox is checked from checklist

I have workflow screen associated with each transition. Mainly transitions have checklist. 

I want a textbox, which will appear when I check any checkbox.

Can you please let me know the plugin. I have already installed Script runner. I am not able to find the solution. I am using jira6.1.

Please help me in implementing the same.

Or if there is any solution using scripts, Please let me know that also.
