I don't think there is any in-built feature except going for marketplace apps. May I know the purpose of creating such forms to guide you?

We are using confluence also to detail small ad hoc projects (such as demo to customers) which involves several teams work preparing to it. The problem is our project managers are indiscipline therefore they neglect mentioning many things that other teams need to know in order to properly prepare.
I though to overcome that fact by creating a form where they have to fill in all the textual fields and check the proper checkboxes.

Let me chime in into this conversation as the area of forms in Confluence and Jira is something I know very well (developing and supporting our ConfiForms app for many years)
The closest you can get (without any extra plugins) is to create a Confluence template with variables or predefined task lists and ask your PMs to create pages from this template filling in the blocks you have defined
