Create Form button not available

I have a team managed project that includes a single Form.  I'm attempting to create a second form, however the Create Form button is not visible from the Forms Tab like i would expect from this help site: https://support.atlassian.com/jira-work-management/docs/how-to-create-edit-save-and-delete-a-form/

Do you mean the "submit" button on the form? If so, that should be automatically added when the form is created. You can try checking using the preview button.

If you meant a button to create the form, the form is automatically saved, so you don't have to manually save the form.

Thanks for the thoughts Mervyn, 

I have One form already created.  I'm trying to create a second form within the same project.  From the technical note it sounds like there should be "Create" button on the right panel of hte forms page to allow for creating a new Form.  That doesn't appear for me.  I was hoping to identify why that is not there?
