Is there any apps can provide Evaluation mechanism function for Jira Software?

For example, I can define an evaluation form with the evaluation item and the grading by myself. Also, I can define what action will trigger Jira to send the evaluation form to my team member automatically.

That sounds like a good use for Jira, but I'm not sure what you mean by "evaluate mechanism".  If you mean the code that goes into an application to enable evaluation periods, that has nothing to do with Jira.

My assumption is that you really mean you want to track people evaluating your product - everyone you give an evaluation licence/key/software too would be created as an issue, with fields for tracking the start/end date, their feedback, and other stuff like who they are and how many users etc.  When an evaluation reaches a certain time, you'd send something to people to ask them about their evaluation, and probably record that on the issue.  (I wouldn't create an "evaluation form", I'd ask them to put it straight into Jira)

I'm not aware of any apps that would help you with building that.  But that is just a guess at what process you are trying to model, and if it's wrong, could you maybe explain what you are trying to model?

The evaluate mechanism is like a questionnaire. I can decide what questions want to ask and how to calculate the grading, and send the questionnaire to certain people.

For example, a group joined to develop a function. After they finished, leader can send the questionnaire to everyone to let them grade for themselves and grade for other people. After everyone finished the questionnaire, system can sum up the result.
