I know it's possible to display a screen during a workflow transition and have comment as a required field on the screen, but instead of a workflow transition, if we are just changing the assignee, is it possible to force JIRA to display a screen so users can comment, or another way to just force the user to provide a comment? Any add-on that does this?
This is for JIRA Cloud.
It is not possible out of the box however one workaround will be to remove assignee field from edit and view screens.
Add a workflow to change or modify assignee. Add a transition screen and make the fields mandatory.

Thanks for the response Prakhar. Are you referring to transitioning from a status to itself? I didn't realize this was possible but then just tried it now and turns out it is. So with that transition, you can display the screen with the comment field mandatory.
However, you still wouldn't be able to force this when you click on the 'Assign' button in the issue details page, or even if the issue is created without an assignee and you click on 'Assign to me'.

This part is little tricky. You can remove assignee field from your edit and view screens.
I believe this should take care of 'Assign to me'
For Assignee button you can do below :
In your Manage Add-on section , search for plugin "Issue Operations Plugin". In its module you will find
module . You can disable it. But be aware that it will remove the assign button for all projects since this operation is system wide.
