Putting a form in the comments for the costumer

Hello community.

I don't know how to put a form in the comments, so that the solver can say if the issue is solved or not.

I mean, that a form only appears when the assignee of the issue wants to see it or in a specific state.

I have other question. We can do it this when we put in a transition, I mean, if my issue is in progress and the development team finish their work and they move the issu to DONE, can I ask to costumer if this is true? but ask automatcly, not manual?
