How do I configure my excel CSV so that I can import story type issues and their subtasks?

How do I indicate in the csv that the story is associated with an existing epic and then how do I indicate the the subtasks are associated with the story? What columns in my file show these associations?

These instruction do not describe importing a New Story with new sub-tasks hierarchy. Is this possible?

I also have a need to import a list of subtasks to an existing Jira story. In this case do I use the Parent ID field to associate new sub-tasks with an existing Jira story when imported?

Unfortunately, this documentation  does not describe importing an Epic, Story, Subtask hierarchy.
