I think you are totaly missing the important information.
What do you mean by "module" and what do you mean by "test plan".
Any of them are not default names of jira. So say more information since nobody will answer.

Ok, so you have install Xray addon?
I think the right place is not here in general question for jira. Use their support system they are very quick and helpfull.
Because we also use Xray I can tell you my experience. Again you do not write from where you would like to import.
If you have some CSV with tests or you have tests in excel then you can use xray importer and import test into jira.
Then create test plan and add all test into test plan by link them at once.
So you can:
1. import tests from CSV, create test plan and add them
2. import tests from CSV and set up the test repository path, then create test plan and add tests from this folder
3. import tests from csv and set up the test plan associated with this test (field name "Test Plans associated with a Test")
4. import tests from CSV and set up test respository path and then in the repository under some folder mouse right click and choose the Create Test Plan Here and it will create new test plan with all tests under this folder optionaly with the same folder structure.
5. etc.
