Unable to create Issue

I have admin access to the application and to the project but unable to create issue.

UPDATE : Just tried in EDGE worked fine, can't do in Google Chrome

Do you have any extensions running in Chrome? Have you tried using an incognito window to see if that works?

I have few extensions, I tired in incognito mode and it worked perfectly. Why it is not working in normal mode.

Most likely it is one of your extensions that is interfering, the only thing I can recommend is to try and find out which one by disable/enable each of them.

Found it, its my Note book web clipper extension. Lucky I got in 3rd attempt.

I experienced this same issue. The culprit was the same extension - Notebook Web Clipper.
