Transition failed; Internal server error while changing status to closed

I am facing a 'Transition error, Internal server error' error everytime I am trying to change the status to close the ticket. The transition in 'Validated in Production' in most of the cases. I have tried everything which other queries on this platform suggests, and changed the workflows too to make it work, but still facing the same error.
Attaching ss of Post Functions, if it might have an issue.
Please suggest what should be done.

Hi Mayank - Welcome to the Atlassian Community!

Not sure that it would matter, but does the Released to Production status get the Resolution set when an issue is transitioned to it? 

And I assume it is the Assignee who is trying to execute the transition? 

Can you post the Exact error that the user gets? (preferably screenshot)

Don't see anything, that can cause such behavior in postfunctions. Can you show us, what is in Conditions?
